••••• Refunds policy ••••• A 80% refund will be issued if notification of cancellation is made to College of Pharmacy Continuing Education by September 2, 2018. If payment of registration was made by check, please allow up to 60 days for refund. Refunds will not be made for cancellations received after September 2, 2016; however, substitute registrants will be accepted. ••••• Accreditation ••••• You have only 60 days from the session completion to received your CE hours. Requests made for CE hours after the 60 day limit can not be honored or per ACPE guidlines. Avoid this by ensuring your NABP# and D.O.B. are correct in your profile, complete the post session evaluations, and check you NABP profile to ensure you receive the hours. It is your responsibility to ensure you receive the CE hours. ••••• Video/Audio/Photography/Exhibitor Release ••••• I authorize UT College of Pharmacy, CE to release my name and email to exhibitors upon request for explicit use of follow up contact only, to record and include in the recordings of the sessions and related materials my name, likeness, image, and/or voice. •••Attendee Information••• Attendee lists will be shared upon request which would include only name and place of employment if entered in the registration. Please note, as a State operated program (FERPA), participation in this program means your category I information may be shared upon request which may include confirmation of attendance, name, address, and contact information.

Brief Bio : James A. Karboski, Pharm.D. is a Clinical Associate Professor of Pharmacy and Assistant Division Head in the Health Outcomes and Pharmacy Practice Division at the University of Texas at Austin College of Pharmacy. He holds the Neill B. Walsdorf Fellowship in Pharmacy. Dr. Karboski earned a Bachelor of Pharmacy degree from Washington State University in 1985. Subsequently, Dr. Karboski earned his Doctor of Pharmacy degree from the University of Texas at Austin and the University of Texas Health Sciences Center at San Antonio in 1987.
